A CCD or Continuity of Care Document is a constraint on the HL7 standard. It specifies that the document must consist of mandatory textual information as well as optional pieces for software processing. A CCD is an XML-based markup that specifies structure, encoding and semantics of a patient summary clinical document for exchange.
